Cochrane Systematic Review: Reishi for Cancer Immune Support

Based on articles retrieved from PubMed, this Cochrane review — the gold standard of evidence synthesis — pooled 5 RCTs in cancer patients. Reishi used alongside conventional chemo/radiotherapy made patients 1.27 times more likely to respond positively compared to conventional treatment alone. Reishi significantly increased CD3, CD4, and CD8 T-lymphocyte percentages (by ~2–4% each), with marginal increases in NK cell activity and meaningful quality-of-life improvements. Reishi alone did not demonstrate significant tumor regression. The compound was well-tolerated across all trials, with only minor adverse events (nausea, insomnia in isolated cases) and no major hepatological or hematological toxicity. Dose note: RCTs typically used 1.5–3 g/day of Reishi extract. This supplement's 150 mg is approximately 10–20 times below these studied doses. This is a meaningful gap, though the degree to which effects scale linearly with dose has not been formally established in humans.

Well-Designed Australian RCT: Reishi Shows No Effect on Metabolic Syndrome

This prospective, double-blind, randomized, placebo-controlled trial (n=84, type 2 diabetes with metabolic syndrome) tested 3 g/day of Reishi (with or without a second mushroom extract) for 16 weeks. Assessed as low risk of bias by multiple independent tools, this is considered one of the most methodologically rigorous Reishi trials in the literature. The combined Reishi group showed no significant improvement in any primary outcome (HbA1c, fasting plasma glucose) or any of the secondary outcomes (blood pressure, triglycerides, waist circumference, BMI, CRP, cholesterol, or quality of life) compared to placebo. The treatment effect on HbA1c was close to zero (0.13% increase favouring placebo). Adverse events were mild and distributed similarly across groups. The authors concluded that further research on Reishi for metabolic syndrome is difficult to justify without evidence of even a modest trend toward benefit.
