Sugar-Free Gum (Xylitol) and Plaque Quantity: Systematic Review and Meta-Analysis

This PROSPERO-registered systematic review and meta-analysis of 8 RCTs found that sugar-free gum — predominantly xylitol-based — significantly reduced dental plaque quantity compared to no-gum controls (effect size −0.778, 95% CI −1.167 to −0.39). A separate analysis of xylitol-only gum studies confirmed the same finding (effect size −0.743, p<0.05). The mechanism is well established: unlike fermentable sugars, xylitol is not metabolized by oral bacteria (especially Streptococcus mutans), starving them of energy while also stimulating salivary flow and delivery of antibacterial lysozyme. One included study reported up to 21.58% reduction in plaque accumulation, and a 2-year RCT showed xylitol gum significantly reduced plaque index in children.

Antibacterial and Anti-Adherence Effects of Plant Extract Mixture Including Psidium guajava

This laboratory study used an artificial mouth model to test guava (Psidium guajava, "sp.") extract individually and in combination against the key oral pathogens Streptococcus sanguinis (early plaque coloniser) and Streptococcus mutans (caries-causing late coloniser). Guava extract alone demonstrated synergistic antibacterial activity in mixture, and the plant extract combinations showed significant inhibition of bacterial adherence to the experimental pellicle — the first step in dental plaque formation. The active compounds (phenolic acids, guaijaverin flavonoid) in guava interfere with bacterial adhesion receptors, reducing biofilm formation. MIC values for guava extract against S. mutans were within range for antimicrobial effect, and SEM imaging confirmed reduced bacterial populations on treated surfaces.
